The SCAN Family & Friends Program

SCAN has designed this unique, free program to keep a relative or close friend of a SCAN member informed about important benefits updates and general health information. SCAN Members can authorize one family member or close friend to participate in the program. Once SCAN receives a the request to join, the relative or friend will begin receiving important SCAN benefits and health related information throughout the year.

To register for the SCAN Family & Friends Program, call SCAN at 1-800-559-3500 or click the button below to register online. Any designated family member or friend can fill out the card. SCAN members will need to complete and sign the card to finalize enrollment.
